MEDIA RELEASE Your parent must sign this agreement if you are under the age of 18. I hereby agree and give my permission to the FillUP enrichment education 2024 summer junior tennis tournament (the “Foundation”) and/or its partners to record, film, photograph, audiotape or videotape my/my child’s name, image, student work, and performance (hereinafter collectively referred to as “Works”) and to display, publish or distribute these Works for the purpose of publishing, posting and/or broadcasting on various media as determined by the Foundation. I hereby waive any right to approve the use of these Works now and in the future, whether the use is known to me or unknown, and I waive any right to any royalties related to the use of these Works. I understand that the Works may appear in electronic form on the internet or in other publications outside of the Foundation’s control. I agree that I will not hold the Foundation responsible for any harm that may arise from such unauthorized reproduction.

INJURIES AND DAMAGES: In consideration of my child and/or children’s attendance and participation at this 2024 Junior Tennis tournament (Tournament) , I, the undersigned (hereinafter the “Releasor(s)”), WAIVE AND GIVE UP LEGAL RIGHTS TO SUE OR CLAIM COMPENSATION FROM THE Tournament, AND ITS DIRECTORS, OFFICERS, EMPLOYEES, VOLUNTEERS, CONTRACTORS, AND AGENTS (COLLECTIVELY CALLED THE “RELEASEES”) FROM ANY AND ALL CLAIMS FOLLOWING AN ACCIDENT. I, the Releasor(s), hereby acknowledge that certain risks of injury, loss or damage of properties are inherent in participation in the sports and/or other activities, including negligence of other persons and negligence on the part of the Releasees. I understand that negligence includes failure on the part of the Releasees to take reasonable steps to safeguard or protect me, my child and/or my children from the risks, dangers, and hazards of the School activities. I willingly agree to assume responsibility and liability for those risks as a condition of registering myself, my child, and/or my children for the program at the Tournament. I acknowledge that there may be minor injuries such as bumps, bruises, cuts, insect bites or insect stings, concussions, broken bones, as well as the risk of serious injury or death. I acknowledge and understand these risks and grant permission to the employee of the Releasee(s) to call emergency assistance when required. I, the Releasor(s), willingly agree to hold harmless and indemnify the Releasees from all liability for any property damage, loss or personal injury to any third party resulting from myself, my child’s and/or my children’s participation in the tournament activities or use of the equipment or facilities or travel beyond the tournament boundary.
